the veteran has smoked the equivalent of at least 10 pack-years of cigars before the clinical onset of the acute myeloid leukaemia, and where smoking has ceased the clinical onset of acute myeloid leukaemia has occurred within ten years of cessation.

the veteran has smoked the equivalent of at least 15 pack-years of cigars before the clinical onset of the acute myeloid leukaemia, and where smoking has ceased the clinical onset of acute myeloid leukaemia has occurred within five years of cessation.
