The referral letter to the psychiatrist should include:
- details of the type of service, including dates and places;
- the condition as claimed (in their own words); and
- the contention.
The letter should request that the psychiatrist provide the details set out in paragraph 2 of Part 1 of these guidelines. A copy of the relevant authority for the release of information should be provided.
Draft letters are available for MRCA, VEA and DRCA claims and you can modify these letters to suit the claimant's circumstances.
