Once the Rehabilitation Provider has been chosen it is essential that the DVA Rehabilitation Delegate make contact to establish open communication and dialogue with the Rehabilitation Provider.

This first and less formal communication between the Rehabilitation Delegate and the Rehabilitation Provider will:

  • enable the Rehabilitation Provider to confirm their capacity to take the referral;
  • highlight the need for any specialist service providers that may be needed for this client;
  • allow the Rehabilitation Delegate to flag or clarify with the Rehabilitation Provider any particular issues or possible barriers that have been identified with this client.

Using the appropriate documentation, a formal written referral is sent to preferred provider.
