REQUESTS FOR INFORMATION

1.The purpose of this Defence Compensation Instruction is to advise where requests for information on individual Service personnel are to be sent.

Cancellation of Defence Compensation Advice

2.The following Defence Compensation Advice are cancelled:

a.Defence Compensation Advice No 56 - Request for Medical Records from Director of Army Health Service, Health Records, Melbourne;

b.Defence Compensation Advice No 71 - Requests for Service Medical Documents from ADF Health Services; and

c.Defence Compensation Advice No 72 - Requests for Information from Navy Office.

Policy

3.To ensure that requests for information for employment or medical related issues are actioned efficiently and effectively, it is essential that requests are directed to the correct area in the first instance.

4.All requests for information on financial conditions of service (including salary and allowances, service conditions, financial entitlements and benefits), policy matters, contentious matters and matters requiring guidance and direction, ADF sport and physical fitness, matters requiring a co-ordinated reply from several command and/or staff areas within the ADF and research of service history records for guidance on complex matters not able to be addressed in simple records inquiries for all personnel in the ADF are to be sent to:

9.All requests for information from the above agencies must contain the following details:

  1. client's name (surname and given names, and any name changes);
  2. service number or numbers;
  3. rank;
  4. whether or not member is serving or discharged and, if discharged, the date of discharge;
  5. dates of claimed injury (if available);
  6. dates of claimed cause of injury (if available);
  7. clear details of the client's claims and/or a copy of the client's attributing statement; and
  8. regional office file reference and contact officer.
  1.                 If the request for information is genuinely urgent, the reason for the priority should be clearly conveyed.  Should regional offices have concerns regarding the time taken to respond, details of the concerns are to be forwarded in writing (accompanied by all supporting documents) to DMCR.

Amendment Action

11.Remove DCI No 29 Amendment 2 dated October 1998, and replace with DCI No 29 Amendment 3.
