A place referred to in s6(1)(b)(vii)
This option is to be chosen where the client is at one of the places referred to in s 6(1)(b)(vii) of the SRC Act, ie. a place where they are:
- obtaining a medical certificate;
- receiving medical treatment;
- undergoing a rehabilitation program;
- receiving a payment of compensation;
- undergoing a medical examination or rehabilitation assessment;
- receiving money due to him or her.
The option “None of the above” should be chosen if the place was not one of these places, a place of work or place of education, or involved travelling.
