A place referred to in s6(1)(b)(vii)

This option is to be chosen where the client is at one of the places referred to in s 6(1)(b)(vii) of the SRC Act, ie. a place where they are:

• obtaining a medical certificate;
• receiving medical treatment;
• undergoing a rehabilitation program;
• receiving a payment of compensation;
• undergoing a medical examination or rehabilitation assessment;
• receiving money due to him or her.

The option “None of the above” should be chosen if the place was not one of these places, a place of work or place of education, or involved travelling.
