You will be required to complete sections 1 through 4 and take the form to the specialist appointment. The specialist will then consider your responses and may discuss them further with you before providing his opinion in relation to your condition.
In some instances, Comcare will arrange for an interview to be conducted for completion of your part of the form. If this is necessary, you will be advised.
Comcare will meet the cost of the specialist consultation.
