- Lodgement of an Initial or Deemed Claim
Why an initial claim is important
When a person declares to DVA their intention to claim an income support pension either in person, in writing or by telephone, the grant of pension may be backdated to that date in certain circumstances. Therefore the initial claim must be recorded, the person provided with the relevant proper claim forms and asked for proof of identity and other documentation.
Form D0501 Intention to Claim Income Support Pension
If a person declares their intention to claim an income support pension complete the D0501 Intention to Claim Income Support Pension and follow the steps below:

If, on the day, the person appears to be eligible or will be eligible within three months, go to step 3. If, on the day, the person does not appear to be eligible, and will not be eligible within three months, then set a reminder to recontact them, or ask them to recontact DVA to ensure that a valid initial claim has been lodged from the earliest date of eligibility (eg the veteran’s 60th birthday). |

Advise them that lodging the proper claim within three months of their initial claim will ensure that DVA pays them from the earliest date they are eligible after the date of the initial claim. |

In the Office Use section of the D0501 form complete the boxes confirming which forms and fact sheets need to be sent or given to the person. Give the person the relevant Claim Forms, Supplementary Forms and Fact Sheets, or prepare a package to send. |

If the person is present, they must sign and date the Intention to Claim Income Support Pension Form acknowledging receipt of the Forms and the requirement to lodge the issued Forms within 3 months to ensure the grant of pension from the earliest possible date. |

In the Forms Issue section on the Intention to Claim Income Support Pension Form complete:

If sending the claim forms and fact sheets, access the Income Support Standard letters [New Claim] and select the letter {Cover letter for IS package] and complete the details to produce this letter. Sign and date the [Cover letter for IS package], take a copy of the letter for DVA records, and send with the forms and fact sheets. |

Complete a File Minute of any other points discussed with the person during the interview/conversation and attach to the completed form D0501. If contact is made by telephone record the phone call in aDVAnce. |

The Form D0501, [Cover letter for IS package] plus any other documents from the interview/conversation should be combined and stored in accordance with local procedures. |
