CLIK system error report.

This form is for reporting CLIK system errors.  Content errors and requests for content changes should be directed to the responsible business areas.  If this form is not working, please send your error report by email to CLIK@dva.gov.au.  Please submit the form once for each person or each error.

Instructions.

1. In the first field, paste the URL (web address) of the page you were using when the error occurred.
2. In the second field, provide a detailed description of what you were doing, what should have happened, and what did happen.
3. In the third field, upload a screenshot of the error if possible.  Press the Print Screen button, copy the image into MS Paint, and save it as an image file.  Remember to hit the Upload button.
4. Click the Submit button.
