In the SOPs for anxiety disorder, among others, the RMA defines 'a clinically significant psychiatric condition' to mean "any Axis 1 disorder of mental health that attracts a diagnosis under DSM-IV-TR which is sufficient to warrant ongoing management, which may involve regular visits (for example, at least monthly), to a psychiatrist, clinical psychologist or general practitioner".
Axis 1 covers clinical disorders, for example, substance-related disorders, mood disorders, anxiety disorders (including PTSD), depressive disorders, bipolar disorder, and schizophrenia.
Generally it would be expected that if a psychiatric disorder warranted treatment then treatment would be sought. However, where treatment was not sought, a person's psychiatric disorder might still warrant treatment if there was persuasive and professional evidence, based on a comprehensive clinical history, that ongoing management had been warranted.

(1) As a cause of depressive disorder, this SOP factor applies only for:
The causal factor requires "having a having an endocrine, cardiovascular, respiratory, metabolic, infectious, or neurological disorder, where the general medical condition is a direct physiological cause of the depression at the time of the clinical onset of depressive disorder".
RMA definition
"the general medical condition is a direct physiological cause of the depression means signs or symptoms of depressed mood are directly related to the pathological process of the general medical condition, and:
(a) the depressive symptoms have a close temporal relationship with the onset or exacerbation of the general medical condition, and the depressive symptoms developed at the same time or after the onset of the general medical condition;
(b) treatment which causes remission of the general medical condition also results in remission of the depressive symptoms; or
(c) features of the depressive disorder, such as an unusual age of onset, a qualitative difference in symptoms, or disproportionately severe or unusual symptoms, are inconsistent with a primary diagnosis of any of the mood spectrum disorders".
The application of this factor is very much a diagnostic issue and the psychiatric report that conforms with the Repatriation Commission Guidelines for Psychiatric Compensation Claims [4] will include details of the general medical condition.
Directly related to the pathological process of the general medical condition means, for example, that the medical condition causes an excess or deficiency of a hormone, metabolite or electrolyte that impacts on brain function, or alternatively, directly effects neurological function. Examples include:
(2) As an aggravating factor for depressive disorder, this factor applies to:
SOP factor requirements
The aggravation factor requires "having a medical condition as specified at the time of the clinical worsening of depressive disorder".
RMA definition
"a medical condition as specified means an endocrine, cardiovascular, respiratory, metabolic, infectious, or neurological condition, that causes symptoms consistent with depression, as a direct physiological consequence of the condition".
General information
The psychiatric report that conforms with the Repatriation Commission Guidelines for Psychiatric Compensation Claims [4] will include details of any medical condition that causes depressive symptoms as a direct physiological consequence.
A direct physiological consequence of the condition means, for example, that the medical condition causes an excess or deficiency of a hormone, metabolite or electrolyte that impacts on brain function, or alternatively, directly effects neurological function. Examples include:

The SoP factor is 'having a medical illness or injury which is life-threatening or which results in serious physical or cognitive disability'. The Repatriation Medical Authority (RMA) has not defined this factor.
Whether a condition is life-threatening can be a difficult assessment, particularly where cancer has been diagnosed in the past but is currently causing little, if any, incapacity – for example a man with cancer of the prostate that attracts only 10 points under GARP for urinary problems. It can be argued that cancer of any type (except perhaps skin) is always a serious disease, but in this case it is clearly not a serious physical or cognitive disability. But is it "life-threatening"? An assessment in terms of the Life-Expectancy table in GARP can help. The phrase "normal, or near normal, five year survival" used in table 14.2 of GARP means that statistically there is little chance of dying from the disease in the next five years. It doesn't mean the person will remain disease free, but if the person is disease free at five years after diagnosis or staging then there is little prospect of disease recurrence. To a certain extent "life-threatening" is also a matter of perspective. An 80 year old with low grade prostate cancer does not have a life threatening disease. A 50 year old with aggressive prostate cancer does.

A sleep disorder means a dyssomnia, a sleep disorder related to another mental disorder other than depressive disorder, a sleep disorder due to a general medical condition or substance-induced sleep disorder, as defined in DSM-IV-TR.
DSM-IV-TR states:
Dyssomnias are primary disorders of initiating or maintaining sleep or of excessive sleepiness and are characterised by a disturbance in the amount, quality, or timing of sleep. This section includes primary insomnia, primary hypersomnia, narcolepsy, breathing-related sleep disorder (includes sleep apnoea), circadian sleep disorder, and dyssomnia not otherwise specified (includes insomnia due to environmental factors, and restless legs syndrome). Parasomnias, abnormal behaviours associated with sleep, are not dyssomnias.
A sleep disorder related to another mental disorder other than depressive disorder, involves a prominent complaint of sleep disturbance that results from a diagnosable mental disorder, but that is sufficiently severe to warrant independent clinical attention.
A sleep disorder due to a general medical condition involves a prominent complaint of sleep disturbance that results from the direct physiological effects of a general medical condition on the sleep-wake system. An example of a direct physiological effect would be pain from a musculoskeletal disease.
A substance-induced sleep disorder involves prominent complaints of sleep disturbance that result from the concurrent use, or recent discontinuation of use, of a substance (including medications).
Whilst these conditions are diagnoses under DSM-IV-TR they do not need to be confirmed by a psychiatric specialist.
37429 there is some evidence that a sleep disorder may be a factor in the development or worsening of the condition under consideration.

The RMA has defined acute cholinergic poisoning as:
"symptoms and signs due to the inhibition of acetylcholinesterase enzyme activity which occur within twenty-four hours following exposure. These symptoms and signs are acute paralysis, overwhelming bronchial secretions, bradycardia, gastrointestinal distress, miosis, lacrimation or diarrhoea."
For the purposes of the SoPs for Depressive Disorder, Parkinson's disease and parkinsonism, acute cholinergic poisoning is by exposure to an organophosphorus ester.
General information
Acetylcholinesterase is an enzyme that operates in the synaptic clef (the space between two nerve cells) so the next nerve impulse can be transmitted across the synaptic gap. It is sometimes abbreviated as AChE.

As a cause of depressive disorder, this SOP factor applies only for substance-induced mood disorder with depressive features. As an aggravating factor, it applies to all types of depressive disorder. The factor is for drugs that are not known causes of depression, but can cause depression in the individual. Thus there are no examples that can be given.
To apply this factor the history in a particular case would have to match the template as specified in the words of the factor, and the psychiatrist would need to attribute the diagnosis to a specific drug.
The SOP factor requires:
being treated with a drug which is associated in the individual with the development of depressive symptoms during drug therapy, and the cessation or significant reduction of the depressive symptoms within days or weeks of discontinuing drug therapy, where treatment with the drug continued for at least the two days before the clinical onset/worsening of depressive disorder.
39700 there is some evidence that being treated with a drug which was associated in the veteran with depressive symptoms may be a factor in the development or worsening of the condition under consideration.

(c) aromatic hydrocarbon solvents, or
(d) oxygenated organic solvents.
Organic solvents can be grouped into 3 types:
More information can be found in organic solvents.
The SOP factor in depressive disorder applies only to reasonable hypothesis cases and as a cause of depressive disorder, it applies only for substance-induced mood disorder with depressive features. As an aggravating factor, it applies to all types of depressive disorder.
The SOP requires:
inhaling, ingesting or having cutaneous contact with organic solvents, in an unventilated and confined space, on more days than not for a continuous period of at least five years before the clinical onset/worsening of depressive disorder, and where the clinical onset/worsening of depressive disorder occurs within two years of that period.

In the case of depressive disorder, appropriate clinical management may include psychotherapy, group therapy, prescription of psychotropic drugs [including Selective serotonin reuptake inhibitors (SSRIs), Monoamine oxidase inhibitors (MAOIs), and Tricyclic antidepressants (TCAs) or a combination thereof], and lifestyle or occupational changes. ECT [electro convulsive therapy] may be an appropriate treatment in severe cases of depression.
If a person had been unable to obtain appropriate clinical management of depressive disorder, this should be mentioned in the psychiatric report. If not, the person's contention regarding inability to obtain appropriate clinical management should be put to the psychiatrist for comment.

Foetal death in-utero is death prior to the complete expulsion or extraction from its mother of a product of conception, irrespective of gestation, by either miscarriage, induced abortion or stillbirth.
Miscarriage is defined by the RMA as "spontaneous or induced expulsion of the products of conception from the uterus before the foetus is viable". This is generally considered to be up to 20 weeks. Miscarriage is also called a spontaneous abortion. An incomplete miscarriage occurs when some pregnancy tissue remains inside the uterus and a dilatation and curettage (D & C) or vacuum aspiration operation may be required.
The cause of a miscarriage cannot always be determined. The most common known causes of miscarriage in the first third of pregnancy (1st trimester) are chromosomal abnormalities, collagen vascular disease, diabetes, other hormonal problems infection, and congenital (present at birth) abnormalities of the uterus. Chromosomal abnormalities of the foetus are the most common cause of early miscarriages, including blighted ovum.
Stillbirth is the death of a foetus after the 20th week of pregnancy but before delivery. Identifiable causes of stillbirth generally fall into one of three different categories: birth defects in the baby, problems with the placenta or umbilical cord, or maternal illnesses or conditions which may sometimes affect pregnancy.

For the purposes of the depressive disorder SoP, this refers to being in the second or third trimester of pregnancy (i.e. more than 3 months pregnant), or in the 1 year period following childbirth.
Childbirth (also known as labour) is the act or process of giving birth to a child. There are 3 stages of labour - the first stage begins with the onset of regular uterine contractions and ends when the cervix is fully dilated, the second stage is the time from complete cervical dilation to delivery of the foetus and the third stage begins after delivery of the infant and ends with delivery of the placenta.

In the Statements of Principles for Personality Disorder, "severe childhood abuse" means:
(a) serious physical, emotional, psychological or sexual harm whilst a child aged under 16 years; or
(b) neglect involving a serious failure to provide the necessities for health, physical and emotional development, or wellbeing whilst a child aged under 16 years;
where such serious harm or neglect has been perpetrated by a parent, a care provider, an adult who works with or around that child, or any other adult in contact with that child.
It would be difficult for a person with VEA service to meet the requirements of the Statement of Principles factors for either the onset, or clinical worsening of this condition as the person would need to have received the childhood abuse on service, before the age of 16 years, and for this to be related to their service.
The report from a specialist psychiatrist that conforms with the Repatriation Commission Guidelines for Psychiatric Compensation Claims [4] would include a full statement about events (either prior, during or after service) described by the person that may be related to the claimed psychiatric condition.
If there is a reference to severe childhood abuse during VEA service, before the age of 16, such abuse would generally be well documented in the person's medical records, if it was reported, although you may need to specifically request to see the psychological records. There may also be additional special investigative material if this involved a disciplinary hearing, or prosecution of other service personnel.
41513 there is some evidence the veteran's severe childhood abuse may be a factor in the onset of the condition under consideration.
