What to provide the doctor with

The information provided to a doctor where a report is being requested will be dependent on a couple of factors:

  • whether general information only is being sought, or whether the doctor will be required to give an opinion on specific issues.  If an opinion is sought, the doctor should be provided with all relevant information, in order to formulate a reasoned medical opinion;

  • the level of opinion being sought.  In some cases, where a specialist opinion is required, the specialist may need more in-depth information to consider.
