3.9 Case Conferencing
The practice of organising regular reviews with service providers is considered to be best practice in rehabilitation case management. This can provide opportunities for the Rehabilitation Coordinator, the service provider and when appropriate, the client, to jointly review and discuss the case in detail to keep matters progressing and resolve issues of concern.
When major barriers are identified (including for example, treatment, client management or administrative matters) case conferencing involving all key stakeholders, the Rehabilitation Coordinator, the service provider, the client and treating practitioner can be an extremely useful and cost effective mechanism for reaching agreement on moving matters forward and confirming goals or objectives for all players.
Teleconferencing is a facility which can effectively allow all players to participate in such meetings.
