Part C - Notification of Recovery Action

Part C to be completed by DVA

Part C of the clearance request reflects the recovery action undertaken by DVA. It must be completed by DVA and returned to Centrelink within five days, even if no further action is required. Centrelink needs the information provided in Part C to:

  • finalise the status of any possible debt, and
  • close the pending activity in the Centrelink mainframe system.

  • only partially recovered, and the balance of the amount owing is being recovered by DVA; or
  • only partially recovered, and the balance of the amount owing is to be recovered by Centrelink

Adjustment details to be provided

The following must be completed by DVA in Part C:

  • Centrelink contact officer
  • the adjustment amount recovered
  • whom the adjustment amount was recovered from
  • the date of recovery
  • any remaining amount
  • which agency will be taking action to recover the outstanding balance

  • signature (on behalf of Deputy Commissioner)
  • date
  • name
  • phone number
